Breastfeeding elevates prolactin, which suppresses GnRH and can result in lower or inconsistent levels of estrogen, progesterone, and LH/FSH, even if menstruation has resumed.
Therefore, it is at the discretion of the ordering provider to decide whether to have the patient collect a sample while still breastfeeding, with the understanding that hormone levels may be affected, or to wait until breastfeeding has decreased or ceased for a more accurate baseline assessment.
