To ensure the safe and timely return of your sample, please follow these instructions carefully:
Preparation of Shipping Documents:
- Place the shipping label and all signed copies of the Commercial Invoice in the shipping pouch provided.
Labeling the Package:
- Affix the shipping pouch securely to the most visible side of the container. Ensure it is away from any folds or seams. The recipient’s full address should be clearly visible.
Same-Day Shipping:
- Ship your package on the same day as the sample collection completion.
Shipping Options:
- Scheduling a Pick-up: Contact Vibrant Customer Support (support@vibrant-wellness.com or 1-866-364-0963 option 2) to schedule a pickup.
Handling the Commercial Invoice:
- Do not add any additional information (ship date, name, etc.) on the Commercial Invoice. Only your original signature should be present.
Packaging the Sample:
- Use the provided cardboard shipping box specifically designed for stool/urine samples. This helps in reducing damage during shipping.
