Providers can resend necessary questionnaires to patients by accessing the patient order tracking page. Click the relevant product type tab on the order tracking page located on the top left.
Select either option shown below to view the patient questionnaire or resend the questionnaire to their email.
If viewing the patient questionnaire, the patient's name and date of birth are required to obtain access.
