Where do I find my accession ID?

An Accession ID is a unique code assigned to your test kit. It’s essential for logging into the MyWellness Patient Portal and accessing your test information. This unique identifier links you to your specific test kit and results. 

 

Where to Find Your Accession ID

You can locate your Accession ID through the following methods:

  • The card included in your test kit

  • Your collection tubes

  • Email Confirmation

  • Order Summary

 

Accessing MyWellness

With your Accession ID, you will be able to log into MyWellness to view your order summary, kit status, collection instructions, and results. 

There are multiple ways to log into MyWellness:

Email from Vibrant Wellness

  1. Open the email from Vibrant Wellness with the subject line related to your test.

  2. Click the “View in MyWellness” button.

  3. Sign into MyWellness with your username and password. Your Accession ID will be auto-filled. 

  4. If you do not have a username and password, you can use Guest Sign In. Enter your Accession ID, first name, last name, and date of birth. Click Log In.

 

QR Code on Your Test Kit

  1. Locate the QR code sticker on your test kit materials.

  2. Scan the QR code with your phone’s camera. The number next to the QR code is your accession ID. 

  3. Sign into MyWellness with your username and password. Your Accession ID will be auto-filled. 

  4. If you do not have a username and password, you can use Guest Login. Enter your Accession ID, first name, last name, and date of birth. Click Log In.

 

Manual Entry

  1. Go to https://mypatienthubs.com/#/login

  2. Sign in to MyWellness with your username and password. Select the Accession ID associated with your order. 

  3. If you do not have a username and password, you can use Guest Login. Enter your Accession ID, first name, last name, and date of birth. Click Log In.

 

Troubleshooting Tips

If you’re having trouble logging in:

  • Ensure your name and birthdate match what your provider entered.

  • Double-check that the Accession ID is correct and contains no spaces.
