How do Patients Create a MyWellness Account?

Patient Accounts in MyWellness allow patients to create login credentials to access their health information directly. Instead of using a guest login with email links or accession IDs, patients can now sign in with an email and password. A “Forgot Password” option is available to securely reset credentials if needed. The design supports both desktop and mobile access for a consistent experience across devices.

How to Create a Patient Account

  1. Navigate to the MyWellness login page.

  2. Select Create Account.

  3. Enter the required information, including Accession ID, First & Last Name, and Date of Birth.

  4. Enter email and create password, then select Create Account.

  5. Enter the verification code

  6. Log in to the MyWellness Portal with new credentials.

How to Log In

  1. Go to the MyWellness login page.

  2. Enter the registered email and password.

  3. Select Login to access the patient dashboard.

How to Reset a Password

  1. From the login page, select Forgot Password.

  2. Enter the registered email address.

  3. Open the reset email and enter the code

  4. Enter a new password and click Reset Password.

  5. Log in again using the updated credentials.
