You can access your patient's order summary/requisition form from the provider portal:
1. Log into the Provider Portal using the following link
If you don't know your username and password, please contact our support team to reset your password at support@vibrant-wellness.com.
2. Click on the 'Patient' tab at the top of the page.
3. Search for your patient by using the filters.
4. Once you find the entry for the specific patient, click on the button under the "Order Status" column. This will open up the order summary on a new tab. (Note: This button displays the current status of the order.)
5. Another way to access the order summary is from the patient profile. You can access the patient's profile by click on the date of service of the order.
6. Once on the patient's profile, click on the "view" hyperlink to access the order summary.
