Questionnaires can be resent from the issue flag on the Patients tab. They can also be sent from the Patient Profile. Providers can find the questionnaire in the patient profile.
Resending Questionnaires
Open the Patient Profile by selecting the patient's name anywhere in the portal. Then, scroll to the Questionnaire section of the Test Report tab
When the questionnaire opens in a new tab, providers can either complete it on behalf of the patient or copy the URL to share with the patient for self-completion. To access the form, the patient will be prompted to verify their identity by entering their first name, last name, and date of birth.
